Federal Fighter Identification Application
The following procedures have been established for the submission of new Federal Fighter Identification Card Applications to the California State Athletic Commission, Department of Consumer Affairs. NOTE: ALL NEW FEDERAL FIGHTER IDENTIFICATION CARDS SHALL BE ISSUED BY THE CALIFORNIA STATE, DEPARTMENT OF CONSUMER AFFAIRS, DIVISION OF INVESTIGATION.
ALL RENEWALS OF FEDERAL FIGHTER IDENTIFICATION CARDS SHALL BE ISSUED BY THE CALIFORNIA STATE ATHLEITC COMMISSION FRONT OFFICE.
A. Submission of application at Division of Investigation Field Offices. NEW APPLICATION ONLY!
a. No less than forty eight (48) hours prior to scheduled Bout, the Boxer shall telephone the local Division of Investigation Field Office and request an appointment for the submission of application. The Applicant shall arrive promptly at the scheduled appointment time.
i. The Applicant shall provide following documents:
1. A completed and signed Federal Fighter ID application. If the applicant cannot obtain an application before the appointment, a blank Federal Fighter ID application will be provided.
2. A recent passport sized photograph.
3. Two forms of governmental identification (social security card, passport, Student ID, military ID, green card). Non U.S. citizens are required to provide a current passport.

FEDERAL FIGHTER IDENTIFICATION APPLICATION FOR RENEWAL
The following procedures have been established for the renewal of Federal Fighter Identification Cards directly to the California State Athletic Commission, Department of Consumer Affairs. NOTE: ALL NEW FEDERAL FIGHTER IDENTIFICATION CARDS SHALL BE ISSUED BY THE CALIFORNIA STATE, DEPARTMENT OF CONSUMER AFFAIRS, DIVISION OF INVESTIGATION.
A. Submission of application to the California State Athletic Commission office by postal or special delivery mail.
a. The Boxer shall mail to the California State Athletic Commission a Federal Fighter Identification Application Packet to be received by the California State Athletic Commission no less than forty eight (48) hours prior to a scheduled Bout.
i. The Application Packet shall include the following documents:
1. A completed and signed Federal Fighter ID application. A recent passport sized photograph.
2. Two forms of governmental identification (social security card, passport, Student ID, military ID, green card). Non U.S. citizens are required to provide a copy of a current passport. For the renewal of a Federal Fighter ID Card, submission of a previously issued Federal Fighter ID Card is acceptable.
B. Submission of application to the California State Athletic Commission office by email.
a. The Boxer shall email to the California State Athletic Commission a Federal Fighter Identification Application Packet to be received by the California State Athletic Commission no less than forty eight (48) hours prior to a scheduled Bout. The email to be used is federalid@dca.ca.gov.
i. The emailed Application Packet shall include the following documents:
1. A completed and signed Federal Fighter ID application. A recent passport sized photograph.
2. Two forms of governmental identification (social security card, passport, Student ID, military ID, green card). Non U.S. citizens are required to provide a copy of a current passport. For the renewal of a Federal Fighter ID Card, submission of a previously issued Federal Fighter ID Card is acceptable.
