Fingerprint Card Requests
Fingerprint cards should only be submitted if you reside outside of California or are unable to obtain live scan services in California. Fingerprint cards can be obtained by one of the following options:
- Request fingerprint cards (Form FD-258) from your local law enforcement agency. Form FD-258 is the only accepted format of fingerprint card.
- Email the Physician Assistant Board at firstname.lastname@example.org.
- Call the Physician Assistant Board at (916) 561-8780.
When submitting requests by email please include your name and address; requests are completed within 1-3 business days of receipt.
The Physician Assistant Board requires the submission of 2 completed fingerprint cards with the application for licensure.
