Fingerprint Card Mailing Request Form

EIT/LSIT Applicants: Do not use the form below. If you are an EIT/LSIT applicant, you will be mailed two (2) traditional fingerprint cards approved for use in California after you have submitted your application and fees using BPELSG Connect. Do not submit a separate request for fingerprint cards; doing so may delay the processing of your application. Mail the two (2) completed and signed fingerprint cards back to the Board along with a copy of your receipt from BPELSG Connect.

All Other Applicants: Use the form below to request fingerprint cards. Mail the two (2) completed and signed fingerprint cards back to the Board along with your completed application package and fees (including the $49 fingerprint card fee).

FINGERPRINT CARD - Use this method if you reside or are currently located outside of California.

LIVESCAN FORM - Use this method if you reside or are traveling to California to complete the Live Scan process.
